Meet the Founder

Aloha! Iʻm Patrice Akina, a registered nurse in Hawaiʻi with over 5 years of experience in acute care, patient advocacy and care coordination. Throughout my career, I have seen many families struggle to find appropriate care and navigate our complex healthcare system. That experience inspired me to create Kanaʻi Care Management.

I provide independent RN care management services for kūpuna and families across Oʻahu, helping clients navigate healthcare needs, coordinate care, and remain safe, independent, and supported at home.

My goal is to provide peace of mind for families by offering compassionate guidance, advocacy, and personalized support every step of the way. I treat every client with the same care, dignity, and respect I would want for my own ʻohana.
