Services are non-medical case management and do not replace physician care, emergency services, or skilled home health services.
Initial Assessment
A comprehensive in-home assessment designed to understand your current health needs, living situation, and support system. This visit includes a review of medical history, medications, safety concerns, and care goals. Following the assessment, youʻll receive personalized recommendations and guidance to help you or your loved one remain safe, independent, and supported at home.
Basic Care Package
Ideal for individuals who need occasional guidance and oversight.
Includes:
One in-home RN visit per month
Wellness check-in and monitoring
Medication review and organization assistance
Home safety assessment and recommendations
Coordination of appointments and services as needed
Communication with family and healthcare providers, with permission
Resource referrals and caregiver support
Standard Care Management Package
Designed for individuals who need ongoing support navigating health concerns and coordinating care.
Includes:
Up to two in-home RN visits per month
Regular wellness assessments and monitoring
Medication review and organization assistance
Home safety assessments and recommendations
Appointment coordination and follow-up
Communication with healthcare providers and family members, with permssion
Assistance accessing community resources and services
Caregiver education and support
Comprehensive Care Management Package
Our highest level of support for individuals with complex medical needs, multiple providers, frequent healthcare appointments, or families living at a distance.
Includes:
Up to four in-home RN visits per month
Comprehensive wellness assessments and ongoing monitoring
Medication review and organization assistance
Home safety assessments and recommendations
Coordination among healthcare providers, specialists, and community services
Appointment scheduling and follow-up
Regular updates to designated family members, with permission
Caregiver education and support
Advocacy and guidance to help navigate healthcare decisions and changing care needs
